i was on night duty again. hee hee. Had a pretty technically difficult trauma case in the middle of the night with many unnecessary radiation given coz simply too big to penetrate to give a decent looking image for diagnosis . The first round was the most redundant but if i dun do it then i would never find out that particular padding is not radio - friendly at all. It gives too much artefacts to the already difficult to penetrate subject. Sigh... but protocol is protocol ....that is cannot log roll before any x rays were done. i personally dun agreed to that as what's the point of getting the x rays done when i know i would need to repeat them again. But this was an exception as after removing that padding, i still could not get the image quality i want. Upon request, i repeated another round with much higher exposure factors. In my heart, i was praying someone will come and tell me forget about the crappy x rays. But that someone never turned up. Now feeling guilty for zapping so many times. However, there's nothing i could do. Already did my part and informed the clinicians that higher exposure does not guaranteed good images as it will generate more scatter radiation which degrade the image quality further. At least i tried my last attempt to give diagnostic value images. If not, there's nothing could be done to assess the conditions unless go to other modalities. However, given the size,,,,, might not produce good results too. So conclusion is dun get too huge!!!!
Then early in the morning, we have a case of heart attack which was sent to us pretty early. From the ECG, they could detect the high risk factors. Just my first time , seeing someone who willing to give up living in the expense of bearing the cost for the procedure. That's very realistic.. if i could not afford the treatment, then why bother to be treated. Coz after i survived, who is going to pay my medical debts. That's the dilemma ......... Think they spent quite some time trying to persuade uncle to give consent for the procedure which was rather expensive to most. i guess for most will just opt to go through the life saving procedure first then worried about how to settle the payment later even though they might have financial difficulties. Everyone would like to live longer. Well, there are people who think otherwise.
